How can I start the CCNA course with basic knowledge of Excel and MS Word, and no knowledge of A +?

CompTIA A + is an entry-level certificate in the IT field. It's kind of a step 1 to get into the IT field. I don't see how someone without at least the CompTIA A + knowledge level can work in any branch of IT.

No matter what IT field you want to work in, you need at least a CompTIA A + level of knowledge.

Of course, you can study CCNA without having CompTIA A + knowledge level, you will have a little more study to do compared to someone with better knowledge, but you can pass the exam. But when it comes to getting a job, not having an A + knowledge level will be a big deal.

So I recommend reviewing A + and N + before

You don't need A + to study CCNA

CCNA IS AN INTRODUCTION COURSE. With that said, I took it into my shipping year in engineering and had no networking experience. There are many concepts to study, but they are all very basic concepts.
